I give Southern Utah University (SUU) including its employees, board, officers, agents, and assigns full ownership and usage rights of the photographic and video images taken of me (including digital, transparency, print, and/or negative format) in still, single, multiple, moving for video format.

I hereby irrevocably give the SUU my permission to use the images taken of me in any media purpose (except pornographic and defamatory) which may include, among others, advertising, promotion, marketing and package for any product or service. I agree that the images may be combined with other images, text and graphics, and cropped, altered or modified.

I do not claim any ownership, publication, or usage rights of the said photographs/images and authorize the use of these images without compensation to me.

I agree that I have no rights to the images, and all the rights to the images belong to the Southern Utah University Marketing Communication and assigns.

I acknowledge and agree that I have no further right to additional consideration or accounting, and that I will make no further claim for any reason.

I acknowledge and agree that this release is binding upon my heirs and assigns.

I agree that my image used for advertisement, editorial use or other material does not need to be submitted to me for approval and that the SUU artist and assigns shall be without liability to me for any distortion or illusionary effects resulting from publication of my picture, portrait or likeness.

I also irrevocably license and consent to the use of fictitious name, and/or imaginary biographical information about me, in connection with the still and/or motion picture photographs of me for any and all purposes, include without limitation, trade, commercial and editorial usage in any medium new, known or later developed.
