I, blank*, am voluntarily enrolled in Otter for Educational and learning purposes by the Disability Resource Center at Southern Utah University. I understand that this service is provided to me at no cost. Furthermore, I understand that if it is determined that I am not using the service; SUU Disability Resource Center has the right to suspend my access to Otter with the premium access feature. In this case, it will be my responsibility to contact the Disability Resource Center to re-enroll into the service.
I understand that Otter for Education is being provided to me as a supplementary note taking service in order to improve my access to notes in the classroom. If I am receiving a peer note taker as an accommodation, Otter is not a replacement for my Peer note taker services. I also understand and agree to follow up with the Disability Resource Center, when my Peer Note Taker has not supplied notes for 1-2 consecutive class days, knowing that the DRC will provide a different Peer Note Taker as soon as possible.
Students with disabilities who have a note taker as an accommodation have the right to record class lectures only for educational purposes. Lectures recorded in this matter may not be shared with others without the consent of the lecturer. Recorded lectures may not be used for any purpose other than educational purposes for the involved student. Information contained in the recorded lecture is protected under federal copyright laws and may not be published or quoted without the express consent of the lecturer and without giving proper and credit to the lecturer.
