This form activates the accommodation for a note taker in your courses. This form should only be submitted by students who have a note taker as an accommodation and have been registered for accommodations for the term requested. When requesting accommodations for note takers in your courses, please consider the following:
- Be certain that the course will necessitate notes. Courses that are online, practicals, or labs typically do not require notes to be taken.Check the course
- Canvas information, as comprehensive course notes may be supplied by the instructor.
When requesting this accommodation, you can expect the following standards to be met:
- Notes will be delivered via the email you provide us.
- Notes will be provided following each class. (This includes notifications of note taker absences/notes deferred due to course alterations such as cancellations or exam days).
- Notes will be requested to be legible and accessible.
When requesting this accommodation, it is important to acknowledge that this accommodation does not guarantee success in the course. The following are a list of common misconceptions of note taking accommodations:
- Notes will be fully comprehensive of all course material. This means notes should not be expected to include notes from textbook/readings.
- Notes should not be expected to contain all the answers needed for assignment or exam completion.
- Notes should not be considered a replacement for classroom attendance or participation. You are expected to still attend classes and be engaged with the course.
